$15 rebate on your next dental visit with purchase of Oral-B Pulsonic, ProfessionalCare of Sonic Complete rechargeable toothbrush.  This one does not seem to require a dental visit, since the form asks only for the purchase of the toothbrush.

$10 rebate on Oral-B Professional Care or Pulsonic Power toothbrushes.
